Grounding requirement for Outdoor MV (13.2KV) Fusible switch switchgear line up.

My question is relating to the details of installation of outdoor MV (13.2KV) Fusible disconnect switchgear line up in an industrial facility. The Switchgear ground bar, mounting pad re-bars should they all be tied to 4/0 direct buried ground loop with ground rods at each corner of the pad along the perimeter of the Switchgear?
 
Isolated MV substation like this one is supposed to have earth resistance of less than 5 Ohms.
The arrangement explained by you seems good and may be adequate if it can ensure earth resistance of not more than 5 Ohms.
