Fabrication techniques for openings

I am conducting research on the fabrication methods of tanks and pressure vessels, and in particular the methods in which raw openings are created in shells and heads. I am hoping that individuals here that are fabricators will share their common practices. For instance, what are the preferred, most economical methods based on opening size? Is boring or drilling practical for larger openings? How does metal thickness determine the method selected? If flame cutting (if that is the correct term) is used, is the metal affected as a result of the high heat input? Is there a reference text that can be purchased that details the various techniques employed in the fabrication of pressure vessels?
