diaphragm failure

Can some one tell me how can i got this result ?
I'm using response spectrum analys, and i have applied diaphragm constraint. But the deformed shape of the model is unusual as you can see in the picture below

Try reassigning the diaphragms from scratch.

I've Reassign Diaphragm many times, but still the same result. In other Forums they suggest me to check, does every member connected to the same joint. but still have the same result. any other ideas ?
