Cooling Medium Foaming

Our platform is experiencing issues with foaming in its closed loop Cooling Medium System.

Has anyone ever had any issues of this type?

If so, what was the route cause, the short term and long term fix?

Have you checked for leaks into the cooling medium?
 
I seem to recall from a pharmaceutical plant application that the glycol/water coolant suffered entrained air and hence why an entrained air density meter had to be used to monitor concentration.
However, the reason for this may have been linked to leakage across the valves used to feed either steam or refrigerant to reaction vessels causing cross-contamination (and the reason to keep a close eye on concentration - too much water and the refrigerant would freeze!).
