Canadian CSA Standard A23.3 Clauses 13.10.10.2, .3 & .4

CSA A23.3:

Openings in 2 way slab systems without beams I believe similar clauses appear in ACI 318:

Why do clauses 13.10.10.3 and 13.10.10.4 say:
"An amount of reinforcement equivalent to that interrupted by an opening shall be added adjacent to the sides of the opening"

whereas clause 13.10.10.2 say:

"Opening of any size may be located in the area common to intersecting middle strips, provided that the total amount of reinforcement required for the panel without the opening is maintained"

Question:
Why the difference in wording? Why could not the same wording as used in 13.10.10.2 also be used in 13.10.10.3 and 13.10.10.4?

13.10.10.2 is leaving open the possibility that the bending reinforcement may not be directly adjacent to the opening (i.e. you might reasonably spread it over the middle strip

Does that mean that in areas common to intersecting middle strips, if there is a large opening within that area, there will be no bottom reinforcement in the slab for the width of the opening?
