King County, Washington is very strict and frequently on the cutting edge of design. The design manual, compliant with 2005 EPA requirements, is printed both sides, and is 3" thick.

FYI,it is also a pain in the you know what to design to.

Milwaukee, WI and Chicago, IL have achieved mixed results. During heavy rain, the stormwater and the wastewater combine together and are discharged into Lake Michigan. Both cities have completed deep tunnel projects to reduce the possibility of this happening, but it still happens a few times a year.

I live in Milwaukee. Nearly very time we have rain partially treated or untreated sewage is dumped into the lake. We spent $1,000,000,000.00 on deep tunnel is not "deep" enough.
