Where in AISC can you find the distance from a face of a column to the CL of bolt, or is it not a standard?

Not standard with our steel code... assume AISC is similar.

For base plates, I'd lean towards table 14-2 in the AISC manual. This table correlates with allowable construction tolerances in the code of standard practice, which have recently been updated to match ACI.

Given the hole and washer sizes, I'd provide an anchor layout that will prevent the washer from clashing with the column or weld material. (1/2 hole size + 1/2 washer size + weld + at least 1/4")
 
I agree with sbisteel. Celt's reference is good to consider as well, but it's important to remember there's a lot more mistakes in base plate anchorage than there are in steel-to-steel joint fit up.

If you're having to transfer shear through anchor bolts with welded washers, then you may need even more room to get the welds in there.
 
agree with sbisteel as well, the entering/tightening tables are just to ensure they can get the tool in there to tighten the nut still need to think about all the other fit up items.
