ASME VIII App 2 Flanges

Does anyone know or have a good reference for the derivation of the radial, long and tangential stresses (Sh, St, and Sr) in App. 2 of ASME VIII? I can proof everything else in App 2 but want to know how these formulas came to be.
 
The formulas and the constants involved are available in a publication called "Development of General Rules for Bolted Flanges". It is highly theoretical and if I remember correctly shows quite a few differential equations to derive the formulas. If you're looking for the theoretical basis behind the App 2 formulas, this is the book to get your hands on.
