ASME section 9 PQR's

I am having to review all of our weld procedures but have noticed some of our Procedure Qualification Records (PQR) for our WPS Welding procedure qualifications have only 2 test's noted as visual inspection and guided bend tests. My understanding is that a PQR needs to have 3 tests completed. IE (2) mechanical tests (tensile and bend) and a visual test or (1) mechanical (ether tensile or bend), a radiograph (xray) with resulting acceptable report and a visual test.
Can anyone confirm that 3 tests are required or are 2 tests adequate?

If the 3 tests are required I would surmise the PQR's with only (2) tests are invalid?

So for 2" sch 160 pipe with wall thickness of 0.344 seems 2 tensile tests required and (2) root and (2) face bends.
