A quick follow up on this in case anybody else struggled with this, I think it is safe to say the check in 4.7.4 is not a flange alignment check at all. It is a check of how well the spring supports match the piping. This is done before any detailed piping to machine alignment to verify the supports are suitably matched to the piping.
For the second question, flanges are aligned with the supports locked in the cold load position.
I finally got my ITP finished and learned a lot about setting up spring supported piping to rotating machinery along the way.
