My latest 318 is an 02 issue, in the ACI 04 manual. I know there is an 318 (08) model, is there anything in between those years? Where can I found out what is different between those years?

There was an ACI 318-05. They're on a three year revision cycle.

I don't know whether Appendix D appeared in 02; Even if it did, there were some important updated in 08.

Also, I know that the load and resistance factors were recently tweaked; Since I started with 05, I don't know whether 02 has the older load and resistance factors.

Yes..as JedClampett noted, there was an ACI 318-05, done on the noted 3 year cycle, preceding the year of issue of the International Building Code...which they always hope will reference the latest edition of ACI 318, but rarely does!
