Accessible to Wiring

According to the NEC 2017 section 314.29 "Boxes,conduit bodies and handhole enclosures must be installed so the wiring is accessible without removing any part of the building or structure"
If that is the case why do engineers show boxes above hard ceiling (gypsum) with no access panels? I usually witness access panels only where a junction box is located above the ceiling however this code section doesn't state pull or junction boxes. Is this code section stating you can't have any box in an accessible ceiling?

What Keith said.
There must be an access to the attic.
You may have to crawl a long way to the JB.
If there is no access above the ceiling then there must be an access panel below the JB.
An electrician who takes pride in his trade will try to avoid hidden boxes.
Sometimes it can't be avoided.
