1. How many of you got this load combination (1.2 DL + 1.6 LL) exceeded by actual service load?

2. Have you ever used 1.1 DL + 1.3 LL?

3. Do different countries have different kpa load for live load? For residential, office and commercial, what value is in the code of your country?

If your service loading (working or unfactored loads) exceeds 1.2DL + 1.6LL you have a big problem.

Hopefully I have misinterpreted your situation.
 
How on earth can the service load be greater than the factored load?
If the factored load is 1.2DL + 1.5LL (Australia)
then the maximum service load would be DL + LL

If I understand your question.

Yes, just want to know if a load survey was done and it actually exceed the factored loads. However in old buildings, this actually occurred, they added carbon fiber for shear. But do you trust carbon fiber to have strain compatibility with rebars when the diagonal cracking forms and the bars and carbon fiber yield? Has anyone actually done this? I read that some airports and public bridges have this shear carbon fiber for added load (when service loads become close 1.2 DL + 1.6 LL). But does it actually work? It gives me the creeps whenever I take public bridge or visit airports or buildings with this shear reinforcement.
 
The live loads used for design should be such that they only have a very small probably of being exceeded for service loading let alone a factored ultimate condition. If monitoring has taken place to prove that the service load has been exceeding the factored load allowance regularly than the structure has been under designed for it's intended use.

There should be some level of confidence in FRP strengthening because of how extensively it is researched and tested.
