I perform infrared surveys on CMU walls and have found many cracks originate at voids in grouted columns or cells.
I'm not saying this is your problem, but you might want to confirm the wall was constructed properly.

Infrared surveys should reveal where grouted cells are located as well as horizontal bond beams, tip caps, etc.
