My suggestions for your two questions:
1. Insert -> Tables -> Bill of Materials -> Top Level Only.
2. As long as the part numbers are on the top level BOM
and the same part numbers are on the subassembly sheets thats all that matters. The item numbers are meaningless.

You should convince your client to always use part numbers.
In my opinion this is just common sense but you will surely find resistance from them. Part numbers will help you better organize and track. The PDMWorks vault does requires unique file names as mentioned.
