a 100 kva transformer is installed at one of our remote installations. we recently received a report that this transformer is causing fluctuation. can any one suggest for remedy and cause of it

Transformers don't CAUSE fluctuations.  They reflect, from one winding to another, what happens external to them. (Barring an internal fault - which doesn't seem to be the case here)

On which side of the transformer (primary or secondary) are these fluctuations being noticed?

Please describe the type of transformer and system parameter. Here are some basic questions that could help to diagnostic the voltage fluctuation:

1- Is this a standard two winding transformer with or without LTC or a regulator transformer? The best source of information is the transformer nameplate.

2- Register measurement: Min/Max voltage, frequency of fluctuation. Is this a flicker or harmonics that could cause voltage fluctuation? What kind of load is connected to this unit? Ex. large motor starting, furnace, etc.

3- Did you check with your local utility? What is their assessment?
