I've noted in a Publication List (Construction Savvy) a few "construction administration forms" references.  

1.  Construction Contract Forms: CSI
2.  Construction Forms and Contracts.

List doesn't give any info on authors, etc. but the above titles are "ver batim".  

Does anyone have these - used them?????  If so, would they be good in highway/earth construction projects??

CSI is the Construction Specification Institute.  You can find them at www.csinet.org/s_csi/index.asp.  There's also the American Institute of Architects (ww2.aia.org) and The International Federation of Consulting Engineers (FIDIC, at www.fidic.org) and others.  Your choice should be based upon your application after reviewing their products.  Also, I have seen several online bookstores listing publications with such forms and guidance materials.   I have personally used both CSI, AIA and FIDIC documents on many varied projects and find that especially the first two are very comprehensive and readily adaptable to specific project types.
