If I have anchor bolts embedded in a CMU wall (f'm=1500psi)that is fully grouted (grout f'c=4000psi), is it ok to use the shear strength values from 1997 UBC Table 19-D for bolts embedded in concrete, rather than the values listed in Table 21-E for bolts embedded in masonry?  Or do the values for masonry already take into account fully grouted units with higher strength grout?

I think your problems can be best answered by a question also.

Who’s holding the grout?

I will use the values listed in bolts embedded in masonry, even if it is 1,000,000psi grout (just exaggerating).

For your second question, my answer is “ perhaps they already consider a certain strength of grout but maybe not higher “.
