I am converting a bare unfinished room into a music/home theater room.  Dimension: 12 feet by 20 feet with bare concrete floor. floor to ceiling is 9 ft. What materials should I use? for the flooring, walls, and ceiling? How about sound insulation? Any suggested book I should read before starting this project? Thanks.

I would recommend "Industrial Noise and Vibration Control" by J.D. Irwin and E.R. Graf or a similar text.  You should learn a little about sound and how it's "properties" are quantified before you go too far.  Noise control situations involve either isolation of a noise source from a receiver or the absorption of noise by the boundaries of a space.  Your theater room is the latter situation, but if you are in close proximity to a neighbor or have children who need to sleep upstairs you have the former situation.  The sound "quality" of a room is quantified by it's reverberation which is basically the time it takes for a sound to dissipate in the room.  High reverberation leads to high sound energy levels and echo, low reverberation leads to low sound energy levels.  You will probably want low reverberation in a listening room.
