I'm trying to calculate the required thickness for a non-circular unstayed flat cover per ASME VIII-1 UG-34. The formula is as follows:

t = d * sqrt (ZCP/SE)

The formula for a circular flat cover is

t = d * sqrt (CP/SE)

The problem is with the definition of "d". It is defined as "diameter, or short span, measured as indicated in Fig. UG-34, in."

Which is it? The diameter, or the short span? Are the above formulas inconsistant in that they use lower-case "d" to represent two completely different things?

d is the diameter for a circular cover and the short span (or shortest diameter if you prefer) for noncircular covers. In the latter case D is the complementary longest diameter, used to calculate Z: taking d=D you see that Z=1 and you get the formula for circular heads.
