It has four gears on it. I have an idea of what it is in my head and cannot see past it. Any help is appreciated!

Portions of the mechanism shown are rusty enough to suggest it is part of some outdoor equipment of some sort.
As a start, are all four "its" in your OP the same "it" ?

Strong is correct- the large toothed wheel on the left is a chain sprocket (for roller chain). The two gears at opposite ends of the shaft most likely mate with the drive on one end, plus the backup brake on the other.
