I haven't checked the code, but I would assume that this is something that has to be addressed in the design of the pad, not something that is just a shop option.

Yes, but there are requirements. UG-37(h).

it depends on the division .It wasn't allowed up to 2004, but in newest edition(2015,..) it is allowed. On UG-37 (h) there are some requirements like area A5 shall be multiplied by .075 .

Yes, you can split the pad without penalty as long as it is split down the circumferential axis - lowest stress.

sometime you have no option but have to split the pad just in order to install them. but make sure your split line is perpendicular to the shall longitudinal axis, and have a weephole on each split of the pad, and you may want to do bubble test on each split.
