California Building Code 2013 - 1704.4 Contractor Responsibility for Special Inspection

I have a question about Special Inspections per the California Building Code. I work as an engineer for a company that builds and installs pollution control equipment. This equipment is supported on structural steel that we designed. The California Building Code (1704.4) requires a "written statement of responsibility" from the contractor to the building official. Has anyone written one of the these? Would be willing to share the contents that are required? The CBC mentions the following: "The contractor's statement shall contain acknowledgement of awareness of the special inspection requirements contained in the statement of special inspections." Apart from just copying that verbiage, is anything else required?
