I have never seen this with my own eyes, so Im wondering how is this made?
Is this picture about right?
What about model for static analysis? is it Ok?
What about bricks above lintel?

I would appreciate any explanation/photo about this topic,

Could you make the concrete lintel span horizontally at the highest point in the opening and light-frame the angle to bring the opening to its final geometry? Is the opening for a window? If you have to have the angle in masonry/ concrete, you might want to discuss this with the mason to find the best way to construct this.

I would just put steps on the top of the concrete beam to make the transition to CMU easier. It's a little tougher for the formwork but shouldn't be a problem.

I would just slope the bottom, and take the top across level. No need to make it complicated.

No lintel needed if you can take advantage of arching action

If it's concealed, go with hokie66, but if it's exposed, Mike20793's method with thought given to the aesthetics of the steps is going to win the day. You don't give a width but I assume your drawing is roughly to scale with your mortar joint lines. If this thing is 20' wide the hokie method will get a little hokie.
