The laboratory performed face and root bend tests at full thickness, (i.e. 1/2 inch), instead of 3/8 in. as specified on material with greater than 20% elongation. Instead of bending the specimens with at 1.5" diameter, they were bent with a 2" dia. to obtain the same outer fiber elongation. I understand the reasoning for their decision to pursue this route, however I was going to reject them because the Code specifies 3/8 in. thickness per the referenced Figure and QW-462, which references QW-160 for dimensional requirements. Am I correct with this interpretation?

Yes, If the required elongation is provided. the specimens are acceptable. This is a previous interpretation issue and can be viewed here.
