Am Here facing a problem in ASTM A 234 Gr. B Pre insulated FITTING a problem like cavity in parent material. I Suggested to weld and grind it. IT IS OK?

I would evaluate the imperfection to the manufacturing standard....Calculate the tmin. If acceptable, I would leave it as-is, and not attempt to repair it by welding as this will create more problems.

This is an extract from API 5L and would normally apply. So long as you end up with the minimum thickness > nominal minus your manufacturing tolerance it will be ok. Looks like poor QA on the pipe suppliers side, but also looks like a minor blemish to me ( difficult to see what size and depth it is)
C.2 Treatment of dressable surface defects
C.2.1 All dressable surface defects shall be dressed-out by grinding.
C.2.2 Grinding shall be carried out in such a way that the dressed area blends in smoothly with the contour of the pipe.
C.2.3 Complete removal of defects shall be verified by local visual inspection, aided, where necessary, by suitable non-destructive inspection methods. To be acceptable, the wall thickness in the ground area shall be in accordance with 9.11.3.2; however, the minus tolerances for diameter and out-of-roundness (see 9.11.3.1) shall not apply in the ground area.

48" 45 Deg ELBOW SCH STD 9.53mm THK. Depth of cavity is 4mm. ASTM A 234 WELDED STEEL FITTINGS. Working Pressure i don't what is in site. We only supply Pre-insulated.

No. We decided to weld and grinding and PT test.

Now knowing this is a fitting, the only reference I can find to repairs is MSS-SP-75 which limits repair by welding to < 33% of wall thickness - you're at 42%.
Also the weld repair needs to be done to a qualified weld procedure by a qualified welder and then possibly heat treated. Just doing a simple Pressure Test? won't tell you anything about long term stresses or cracking potential.
You can't just get some welder to stick a blob of metal in there and then grind it smooth....

This fitting going to use in CHILLED WATER line is it risk?
Normally i don't suggest repair welding RM. That's my Boss risk.But any how i learned from you.

It's a relatively thin fitting (D/t>100) and depending on how much stress it's under and how many times it flexes as it goes from normal to cold.
In reality it's unlikely to fail, but it's not good practice to do this sort of repair at that sort of depth.

Hopwe you will be advising the Purchaser of what you have done to repair the bend!!! He could well reject the item!!
