Clean extrude...need help

This shouldnt be hard. See attached. All I want to do is have a same diameter cylinder to bridge the gap between the other two cylinders smoothly. I need to keep the same diameter, but have the edges line up when finished. So far Tube and extrude look great coming from one side, but when it hits the other side, the edges don't match up and it leaves a nasty little ridge where they meet. I've tried to make lines to use as a guide for swept, but this leaves a gap. Rules surface lines up perfectly on the right and left sides like I want, but doesn't maintain the diameter. PLease help!
