Am trying place an indoor lap pool on a patio slab on grade.  Pool requires 220lbs/sq.ft.  Pool is 14'-0" x 8'-0".  Slab will be placed over compacted sand to 2500psf.  Will a 6" slab with a mat of #5 rebar 12" OC ea. way, with 6"x6" 10/10 wwm.  be sufficient?  Slab will be monolithic with a footing depth of 12" and a width of 16".  3000psi conc. will be used.

If I understand correctly, you want to place a lap pool of approximately 3.4' deep on a slab.  From your thread, I take it that you are going to construct this slab, so we are not contending with an existing condition.  

That is a lot of steel for a 6" thick slab.  You wont' meet minimum cover requirements for ACI.  Consider designing this as an independent footing and pouring the slab around the footing with expansion joint.  This way you can mitigate slab cracking, reduce costs, and obtain a straight forward solution.  I have reseverations with placing a pool on a slab without some means of isolation, given the nature of loading/unloading (water) and the fact that slab will not be loaded evenly.  If you want a continuous pour, definitely consider at least placing a control joint around and offset from the perimeter of the pool unit equal to the slab depth.

A reference to consider is ACI 360, Design of Slabs on Grade. Also, USACE Design of Slabs Subject to Heavy Loads.  Although your situation does not involve such a detailed analysis, these references should prove quite useful.
