I wonder in what world they figured a floor would support an excavator.

First that bridge and now this. Canada is having a bad year for excavators being involved in demolition accidents.

How did it get up there to begin with?

The sad truth is that it was probably never checked further than applying a uniform load over the equipment footprint. There are a lot of cowboy style contractors in the demolition business -- and a lot of contractors who happen to win a job that includes some demo on the side, and don't want to pay for an engineer.
