I'm looking for a reliable resource for Raised Pattern Floor Plate aka "Checkerplate".

AISC Tables 3-18a & b gives allowable loads. But, is checkerplate typically available in all these thicknesses?

I recommend you contact a local (local to the project) steel fabricator and inquire with them. They would be in the best position to speak about product availability.

If it's a small quantity, call around and see what's available. Typically "Diamond floor plate" (little chevron patterns at right angles) rather than checkerplate (little X's).
