Nobody can really tell: The two plates are welded together, but the tiny joint between concrete (grout sometimes) and concrete "should be" completely closed up by the liquid concrete and steel surface. But, vibration, heat-cooling (from the old welding too!), shock and simple movement of the concrete pad with respect to the structure "may" cause an opening. Water gets in between the two, and rust starts.

And many times, it doesn't. You've got to look in detail at each pad. Try a sealant or good paint over the top of the plates, making sure to create a seal between plate edge and concrete upper surface.

The welding is not specific to this problem - bolted connections can rust too. But less often because the bolt allows a little bit of movement.

The two plates are welded together with an all-around weld, We don't have grout here as this is an embedded plate. But yes, there is a possibility of air gap between the embedded plate and the concrete, IF I introduce grout hole on embedded plate will that help me.

Whether or not corrosion will be an issue depends on the environment (inside or outside, humid or dry, coastal or inland), not so much the weld details.

The site has temperature variation of -40 to +50 degree Celsius, If water penetrates beneath the base plate an if freezes to ice.
Would that cause much upward force on the base plate. IF grout hole was provided in embedded plate will that resolve all these issues.
