The PHMSA people must be getting a little overworked just now. The pipeline incidents per km-year figure looks to be taking a bit of a hit in the US.

PHMSA Overworked. Yes I'd say so and that they've been overworked for years.
Averages work out to only 1 inspector for every 5,000 miles of pipe and 15 pipeline companies.
All of those 3 parallel ex Transco pipelines are getting rather old now.
