What are the factors to consider to determine the cause of failure of boiler steam drum gasket during testing of safety valve at 1450 psi, our normal working pressure is 1350 psi.

Three factors that comes to mind are 1)clean and gouge free surfaces to which the gasket is to be applied 2) suitability of the gasket material for such high pressure 3) do not reuse gaskets, get new ones.
