a) We are designing a repair for a 7½" deep waffle slab of 5" deep rib and 2½" deep slab (5+2½=7½), 19" clear between ribs and 5" wide ribs (at the bottom), sloped sides. This was built in the 1950's. Does anyone know where we can get this oddball waffle form that is 5" deep? Most are 6" deep. The building is in Toronto, but we can bring it in from anywhere in North America. The alternative is to have them specially made, or make them out of wood and place a bond breaker material on the outside, but it would be better if we could find this form somewhere.

b) Also, does anyone know what the radius is for the curve between the rib and the soffit of the top slab?

It sounds like you will need to have some custom forms broke at the local fab shop. Ever thought of doing a 3D laser scan? Very fast, and will get you the exact profile of the existing.

I thought of another idea. If they use a deeper waffle form and fill the bottom of it with wood. For example if the use an 8" deep standard metal waffle form and fill the bottom 3" with wood or other filler, they will have a 5" deep rib, as desired.
