Is there some where in ASME code, specifically Section IV or VIII that says the calculations should have the job number, Applicable Code Edition, Company Name, etc on a cover page or something of that nature attached to the applicable calcs?

That would be good engineering practice. But I don't recall if it is required in ASME.

No. This is left up to the Certificate Holder to demonstrate during a triannual review.

Left to the cert holder. I don't think so. Is a must in every ASME qc assurance.
