I suspect the head area is weaker concrete or grout than below, possibly high W/C ratio. It would seem that all the weak area should be removed and a strong zone of new grout or concrete should replace it. Bond the new to the old with a Portland cement paste applied to dry undisturbed material. Cure the new patch as necessary to gain strength before the test is run.

Projecting part of pile can be more than shown, if needed.

Cut the top of the pile to good concrete. Load test. Then expose rebar and splice to bring up to required level or increase the thickness of the pile cap to accommodate cut.

I agree with oldestguy that pile head is weaker, this should chip to the fresh concrete and rebuilt using the high strength grout.
An 500mm excavation around the pile may be required to get a space to place the dial gauges.
