Anybody has pics of underhung monorail, possible structural problems/failures?

I appreciate any information with possibly stating the cause of the problem.

I do a LOT of monorail design and load rating and have yet to see a true failure. I however have seen overloads and poor design. Can't show any pictures due to NDA reasons but I'll try to describe what I've seen.

Flanges not designed for wheel loads causing bending of the flange to web root.
Dented flanges from forklifts.
Little to no lateral support (which doesn't actually cause failure typically due to the load being below the beam), this can cause extreme deflection issues and serviceability.
Load ratings not taking into account snow/live loads or other loads on the supporting structure(s).
Inadequate column bracing if the ends of the monorail are supported on non-building columns.
Lateral loading (swinging the load or non-centered lifts) not considered causing torsion (this is the only time I've seen anything close to a true failure).
