According to the information I see it would appear that you are looking for the center Ø to be concentric to the outer Ø and maybe perpendicular to the face.

That view is not a good one to specify the dimensions and tolerances. A view showing both concentric circles would be MUCH better. Also, the drafting is pretty sloppy. Are you new to the software? For example, the arrowhead on the leader for the inner hole diameter and positional tolerance should be moved to the other end of the leader at the hole.

I don't see Datum B identified on the part.

I do not understand where you are getting the diameters 4.875-4.874.
I do not understand where you are seeing the 0.002.

Is this the right picture you meant to upload? It is possible I am missing something.

You are missing a lot of information. Also consider that tolerance is a matter of function so without knowing how this is supposed to work, I don't think anyone here can suggest a tolerance value.

Like the others noted, there are two datum A surfaces and no datum B surface shown. You also need to provide a bit more definition of the relationship between the hole and disc, such as a centerline through the hole/disc center. A thickness dimension of the disc would also be helpful, as would something like a profile or flatness tolerance for the disc face used as the primary datum.
