I have a general question on over pressure on Boilers. A 30,000lb/hr PSV is set at 165psi. It is exceed passed 165psi with steam but stays open and is still lifted with water. Now that it has included water, the PSV will need to be retested and certified(or swamped out) ???

How safe is the PSV now...

PSV's are 'positive opening', and may or may not actually reclose tight. But even one that does not fully reclose will still reopen when it sees an overpressure. And yes, that PSV needs to be worked on, by a certified PSV shop.
