i have an equipment block foundation that will require penetration.
is there anything I should be concerned if the block foundation is more than 5 foot thick?

I've done a lot of slab penetration before but not on very thick slab.

You could add reinforcing to make up the bars interrupted, but even that's probably not necessary.

Maybe add 4 bars (# shaped) a few feet long, near the block surfaces at either end of the penetration, just to guard against any cracking from the hole. You’re a little unclear about whether this penetration is top to bottom or otherwise. Is the item embedded in the conc. and tied to the rebar before casting or is the hole formed (cased) in some way and the pipe installed later?

Hole is top to bottom, hole formed and pipe installed later. I guess there is nothing to worry about. I will just add reinforcement around the hole. thanks,

How big is the hole?
How big is the slab?
Use a simple pipe as a form and liner - if the slab is not already poured.
Add two holes. You will need the 2nd one later.
