Given that an installer is installing pressure-rated code-stamped vessels manufactured by a different fabricator who is a code shop, and given that the installer will only be welding interconnecting piping, and its elbows, tees and flanges, and his other work will be the installation of valves and instrumentation...
...must the installer be a "code shop" if the system will be under pressures greater than 15 psig, and ... what's the reference to back this up?

... is the R stamp required to weld piping? I do not think so but I do not know.

Boilers are required to be fabricated to a more regulated standard. In the US, each state will often have its requirements for installation and issuing operational permits.
A "good" field fabrication contractor should (in theory) know all the above.
