The executive summary is well worth the read. The blame is quite well distributed among various parties. A structural engineer occasionally encounters similar situations and it is crucial to know how to behave in such circumstances.

While not a direct comparison, the Save-On Store collapse in Burnaby, BC back in the late 1980's created the current structural engineering peer review process used in BC today. While the Save-On store collapse was a new design - it highlited the typical muddy lines of responsibility, and the broken information chains in the usual building process. The scary thing is are how many more of these potential disasters are hiding around the country that get a superficial review and lack of maintenance money thrown at resolving the aging structures.
