The AGMA has just released several new and updated standards.
The main one of interest is ANSI/AGMA 1010-F14 which is a revision of 1010-E95 'Appearance of Gear Teeth = Terminology of Wear & Failure'.
There are many excellent examples of the various failure modes that gear teeth are subjected to.
