Generally is it a practice that either Insert Plate or RF Pad is used to satisfy reinforcement requirement.

However, is it acceptable if both Insert Plate & RF Pad are used together to satisfy reinforcement requirement?

There is no restriction as per Code but for good industry practice I will avoid RF pad over insert plate. What is the reason for this arrangement? Is insert plate thickness exceeding vertical reinforcement limit?

Yes.
The only option is to change nozzle from Plate Fabricated to Forged SRF. Which will have schedule implications.

Yes, please; a more 'elegant' solution may be possible.

If the insert plate exceeds vertical limits, how will the combined plate and pad not do so? And what is a "Forged SRF"?

If I were the client I would look very unfavorably on this arrangement.
