Is there a limit (code or practical) on the height of a wall that can be poured above a slab that is poured monolithically with the wall?
I'm thinking of a storage tank where they sometimes pour a starter wall monolithically with the slab supporting it, but I'm sure there are other situations that also apply.

Nothing I have specifically seen.

I can see up to a foot above the slab with very little problem, especially considering the stiffness of the design mix used and the rate of the pour used to help the situation.

More than likely you will read (via a groove) the bottom of the form on the inside of the wall though.

For practical reasons, you can't make it too tall. If it's more than a few inches, you'll need to get a stinger in it to vibrate the concrete. And when you do that the concrete will liquefy and disappear.
I've never seen one more than a couple of inches. And if the complaining about them is proportional to their size, that's all you want.
