Does anyone know a good source for examining the failure mode of a bolt head or nut being pulled through a steel plate?

I'm working on a project where A36 plate was substituted for A572 plate.  A basic analysis shows this should be no problem. (The original design was done in Germany where St60 (~A572) seems to be more common than A572 is here)  I would like to read up on the actual failure modes involved for more comfort.

What kind of washers are you using.
Oversize od washers may be in line
to reduce the bearing stresses at
the surface of the plate.
