I have a concrete beam with shears of similar magnitude vertically and horizontally. When using closed stirrups (continuous by way of coupler), can I utilize the vertical legs to resist the vertical shear and the horizontal legs to resist the horizontal shear within the same stirrup based on current code practice?

I think you can use the stirrups in the manner you suggest. For the contribution of the concrete, you could resolve the orthogonal shear into a resultant and use the concrete area only once.

Is there a reason to use couplers? Why can't a traditional closed tie work?

I would actually follow BAretired's advice for the stirrups as well... Size them based on the resultant force of the two combined.
