We have a house built in 1929 that originally had a cistern. Previous owners drilled a passage through the 12inch thick wall so now it's a cold storage room. The wall is 7ft high, and the hole is 42inches high and 16inches wide. It starts 18inches above the floor. If we want to widen the hole by 3inches, and make it 6inches taller, do we need to worry about reinforcing the wall afterwards? Is a lintel required, or will a rounded top be strong enough to handle such a hole?

Any thoughts would be appreciated.

"drilled a passage through the 12inch thick wall" What is the wall constructed of? If it is field stone that is a lot different from concrete. A better description is required. Photos even better.
