I always thought that FULL R required both edges of the radius to be tangent?

Not sure what the .005 MAX FLAT (OPTIONAL) means.

My guess is that the "FULL R" can be such that it leaves a .005 max diameter flat at the center (FULL R +.0000/-.0025?).

I think whoever prepared that drawing was confused.

I have seen "FULL RAD" used to show that the round thing at the end of a slot was a radius with R being half the width of the slot. Today with ASME Y14.5, you must mark it with an "R".

Perhaps the drafter means "SR", meaning that it is a spherical radius of some sort, as per ASME Y14.5.

My guess would be a the intent is an SR with the option to leave a .005 max diameter flat at the center (FULL R +.0000/-.0025?) as ewh suggests, or possibly a flat that is cut back a distance of .005 from the theoretical perfect form tip as another interpretation.
