Where in the ASHTO LRFD Bridge provides an equation for Longitudinal bars requirement in concrete slabs? similar to Equation 5.10.8-1 page 5-121 "shrinkage and Temperature Reinforcement"
Also, the table A4-1 on page 4-98 is it for loading HL-93? what happens for California amendment Design Permit load P15, is it covered in the same table?

Read Chapter 9; note that AASHTO specs allow for traditional design and empirical design. Both have different requirements for longitudinal bars.

Also, the table A4-1 on page 4-98 is it for loading HL-93? what happens for California amendment Design Permit load P15, is it covered in the same table?

Generally, we don't design slabs for permit loads. However, CalTrans may do things differently. To answer your question, read the commentary about how Table A4-1 was derived.
